From test cases it seems that order of notes is not important, though from problem statement what I understood is that chord is formed when notes are played in order base, base+3 (or base+4), base+7 in order (possibly excluding duplicates).
I have no knowledge of music, so my understanding of problem seems incorrect. Could anyone explain what forms a "chord" with respect to this problem?

The order that the notes are listed in, is not important. Think of a chord as some notes being played together.

Yes, it is not important. Moreover notes are repeated with the step of 12 (octave), so that, say:
base, base+3, base+7
is the same chord as
base+3, base+7, base+24
(though it will sound a bit higher)
Surely in the second case you could not at once guess that the lowest note is not base
but base+3
. That is what makes the
small puzzle of this otherwise simple problem :)
